Pre-Participation Requirements

At the start of each year, student-athletes must be cleared by the Compliance Coordinator and Sports Medicine Staff prior to any team activity. Please refer to the content below to prevent any delay in athletic participation. Information below is meant to supplement information sent directly to student-athletes in June of every year prior to anticipated athletic participation. Students must be on an official NCAA roster to be cleared for activity with an intercollegiate team.

Medical Requirements

  1. Log in to Front Rush athlete account
    • If you are unable to log in, first try “forget password”
    • If you still have issues, contact your coach or support@frontrush.com
  2. Complete all appropriate NCAA compliance paperwork
    • Scholar-Athlete Handbook
    • Student-Athlete Statement
    • Drug Testing Consent
    • Hosting Recruit Instructions
    • Student-Athlete Title IX Disclosure and Attestation
    • Summary of NCAA Regulations
    • Athlete Bio Sheet (update every year)
  3. SAPSA Completion
    • Instructions for completion will be sent out via email in late August by the Title IX office and must be completed prior to any competition
  4. International Amateurism and Eligibility Certification
    • Required for incoming or new student-athletes that meet any of the following criteria:
      • Attended a secondary or post-secondary school outside the US, after age 11 (except for US study abroad programs)
      • Your permanent residence is outside the US
      • You or your team was based and competed outside the US, after age 13
    • To Complete your Amateurism Certification please:
      1. Create an Amateurism-only Certification account (option 2) with the NCAA Eligibility Center
      2. Complete the registration process
      3. Request your final amateurism certification through your NCAA Eligibility Center account before Aug 1

  1. Physical Examination and Clearance by a physician (MD or DO credentials) 
    • Physicals must be completed using the Caltech Pre-Participation Physical Examination form
    • Review and complete all history questions, then have physician complete and sign the bottom of each page
    • Ensure the physician or office stamp is placed in the designated area, or a business card is attached
    • Physical exams completed prior to May 31 will not be valid for future seasons

Note: Sportsware has been retired for the 2025-2026 Academic Year: all Pre-Participation Medical Clearance Paperwork should be uploaded via the Student Health Portal available via Access or using this link: https://mycaltechhealth.caltech.edu/login_directory.aspx 

 2. Complete all required items in Student Health Portal

  • Add or Update Emergency Contact
    • Navigate to “Profile” in the left-hand menu 
    • Edit “Emergency Contact” 
    • Provide updated information 
  • Complete the 8 Athletics Forms for NCAA medical clearance
    • Navigate to “Optional Forms” in the left-hand menu 
    • Scroll to find Athletics Forms 1-8 
    • Download, complete and upload each form in the portal 
    • Forms are available in your portal or on the Sports Medicine Student-Athlete Information website 
    • You will be asked to verify each document, please ensure the scanned document is legible, all questions are answered and signatures are visible 
  • Upload Health Insurance Card 
    • Navigate to “Insurance Card” in the left-hand menu 
    • Select “Add New Card” to enter your insurance information and upload a clear photo of the FRONT and BACK of your current health insurance card 
    • If you are a first-year student and you are opting into the Caltech Student Health Insurance Plan, please just enter “Caltech Insurance” Your insurance information will be added to the system in September 
    • If you are a returning student athlete on the Caltech Student Health Insurance Plan, please ensure your insurance information is up to date   
  • NEW student-athletes must submit Sickle Cell Trait results  
    • Please send all Sickle Cell Trait test results to the Nursing Team as a Secure Message Attachment in the Student Health Portal  
    • Results are typically available via your birth hospital or physician 
    • If you need resources for testing, please email sportsmed@caltech.edu  
    • If you are a returning student-athlete, please select the appropriate option on the Sickle Cell Declaration Form (Athletics Form 6) 

3. Complete Baseline Concussion Testing 

  • Baseline Concussion Testing on the SWAY app must be done PRIOR to campus arrival 
  • You will receive an email from the Caltech Sports Medicine staff with instructions for Baseline Concussion Testing (items 1-4 should be completed prior to baseline testing) 

 

Special Circumstances: 

  • If you have had surgery in the last 12 months, you must submit a clearance note from your surgeon stating you are medically cleared for athletic activity, in addition to your completed physical 
  • If you have prescription medication of an NCAA Banned-Drug, you must have a completed Medication Exemption Form signed by your prescribing medical professional 
    • Non-exhaustive list of examples: ADD/ADHD Medication, dexmethylphenidate, dextroamphetamine, methamphetamine, methylphenidate, etc. 
    • Form can be uploaded with your physical or emailed to sportsmed@caltech.edu
