Other Forms and Information

2026-2027 Physical Exam Form : Must be completed by a physician prior to any team activity.
SWS New Student Health Essentials Guide : Student Wellness Services Requirements and Health Essentials for ALL New Caltech students.
Physician Report Form (PDF) : Required for any student-athlete that has been restricted from activity in the past 12months.
NCAA Medication Exemption Form : Required for any student-athlete that as been prescribed an NCAA banned substance by a physician.
    - Non-exhaustive list of examples: ADD/ADHD Medication, dexmethylphenidate, dextroamphetamine, methamphetamine, methylphenidate, etc.
    - Form can be submitted with your physical, uplodaded to your Health Portal, or emailed to sportsmed@caltech.edu
